Overview

We are seeking a compassionate and knowledgeable Hormone Counselor to join our healthcare team. The ideal candidate will have a nursing background and provide expert guidance and support to patients navigating hormone therapy and related treatments. This role requires a strong understanding of medical terminology, case management, and patient-centered care, particularly within clinical settings.

The Hormone

Counselor will play a vital role in improving patient outcomes through personalized counseling, education, and coordination of care. The ability to provide IV treatments is required.

Duties

- Conduct comprehensive assessments of patients’ hormone health needs and develop individualized treatment plans.
- Provide counseling to patients regarding hormone therapy options, potential side effects, and expected outcomes.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
- Educate patients on medication management, lifestyle adjustments, and follow-up procedures.
- Monitor patient progress through regular follow-ups and adjust treatment plans as necessary.




- Maintain accurate documentation of patient interactions, treatment plans, and progress notes in medical records.
- Assist in case management by coordinating appointments, lab tests, and referrals to specialists.
- Stay informed about the latest developments in hormone therapy, acute care practices, and pediatric or hospice care when applicable.

Requirements

- Prior hospital experience or clinical background preferred.
- Background in nursing or related healthcare fields is mandatory.
- Experience with acute care settings or community care is a plus.
- Strong knowledge of medical terminology and case management principles.
- Excellent communication skills with the ability to educate and support diverse patient populations.
- Compassionate approach with a focus on patient-centered care.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Relevant certifications or licenses in nursing or healthcare are mandatory.

Pay: $27.00-$32.00 per hour
