Health is seeking a Licensed Practical Nurse for its Groundspring Primary Care team at Lily Lee Community Health Centre. In this vital role, you will practice in line with BCCNM standards, delivering client and family-centered care. Support for clients experiencing health challenges will include outreach and home-based visits within an integrated care model. Key Responsibilities:

- Collaborate in an interdisciplinary team for client care
- Assess and plan care for primary care clients




- Document evaluations and implement care strategies
- Maintain therapeutic relationships with clients and families
- Seek guidance from RNs/NPs/MDs when required Requirements:
- Graduation from an accredited Practical Nurse program
- Active registration with BCCNM
- One year of nursing experience in holistic care
- Knowledge of Trauma Informed Practice
