SUMMARY OF THE POSITION
A person who, without holding a social work technician diploma, performs work of a similar and related nature to that of a social work technician, within the limits of their competence and training.
The Primary Care Community Representative (PCCR) is a Cree or Indigenous community-based, medically guided paraprofessional who provides culturally safe support to clients and families. The PCCR helps clients navigate the healthcare system, promotes health education, and strengthens communication between the community and the care team. This role bridges cultural and medical perspectives to improve the overall well-being of the community.
SPECIFIC FUNCTIONS:
The Primary care community representative works with other members of the healthcare team to support the delivery of clinical services. The tasks may include:
• Build trust-based relationships with clients and families, assist in navigating the healthcare system, and advocate for culturally protected, timely, and quality care;
• Promote health literacy, disease prevention, and community well-being through workshops, outreach, and educational events;
• Provide counseling and education on diabetes management, nutrition, insulin use, blood sugar monitoring, complications, and mental health support;
• Educate on hypertension, monitor blood pressure, and support lifestyle changes and medication adherence;
• Encourage and support cancer screening (breast, colorectal, cervical) and early detection initiatives;
• Recognize mental health needs, refer to appropriate professionals, promote harm reduction,
and connect clients to social services;
• Support maternal-child health education, assist in well-baby clinics, and promote healthy practices for families;
• Promote safe sexual practices, provide STBBI prevention and education, and encourage screening;
• Participate in school and community health activities, vaccination campaigns, and partner initiatives;
• Maintain bush kits for remote care, and provide services through local CMCs for all age groups;
• Carries out all other related tasks upon the request of her/his immediate supervisor.
REQUIREMENTS
Education:
• High School diploma
• Training as a paraprofessional in community health or willingness to undergo training;
• Foundational knowledge or experience in health education, community work, or culturally based support roles.
Knowledge and abilities:
• Fluent in English and Cree. Fluency in French is an asset;
• Ability to work under supervision of nurses and healthcare providers in clinical contexts;
• Knowledge of Cree culture and social/health issues in Eeyou Istchee;
• Capacity to provide culturally safe, client-centered care;
• Skills in health promotion, counseling, and community engagement;
• Ability to collaborate within multidisciplinary care teams;
• Willingness to learn medical and preventive health concepts.
LANGUAGE
• Fluent in Cree and English;
Other:
• Must be ready to work occasionally outside the regular working hours (availability);
• Possibility of on-call periods.
