Job Details Title: Case Manager Vacancies: 2 Date: April 14, 2026 Closing Date: April 28, 2025 Location: Newmarket, ON - York Region South Simcoe Supportive Housing Compensation: $55,239 - $67,560 Annually Prorated Contract Duration: 6‑Month Contract & 7‑Month Contract Work Type: Onsite Job ID: Existing or New Positions: Existing Job Summary As a Case Manager you will work independently and as part of a team that coordinates a support network and links clients living with mental illnesses, addictions and physical health issues who live in YRSS Supportive Housing to various community services. The position attempts to empower the client, thereby increasing their independence, dignity, and self‑respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. The York Region/South Simcoe Jane Street High Support Program is staffed 16 hours of the day and provides case management, residential and life enrichment supports to adults, aged 16+. The Case Manager works as part of a team, along side Residential Support Workers to assist individuals in meeting their basic needs (e.G., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life. What You Will Do Develop a plan of support and recovery that includes identifying priorities and goals in conjunction with the client, other professionals, and areas identified through initial intake and OCAN’s. Promote personal empowerment by coaching, mentoring and supporting each individual to meet the daily requirements of their living environment; directly assist with daily living activities, especially life‑skills such as hygiene, medication management, cooking, cleaning, shopping and budgeting; develop strategies and a plan for maintaining these activities. Establish and coordinate a support network of social, family, medical, psychiatric and other community links; develop a coordinated care plan as appropriate. Provide crisis intervention and management, and assist in the development of crisis plans with clients. Promote the services of the program and the image of the agency as a representative with clients, families, other supports,



external agencies and others in a professional manner that reflects favourably on the organization. Mediate issues or conflicts between residents, or residents and neighbours, teaching conflict resolution; follow up on issues to ensure resolution. Participate in planning, organizing, promoting and conducting events for the clients to address their social needs and prevent isolation; maintain general awareness of community events and activities to meet these needs. Participate in program administration by completing and maintaining required records, assessments or statistics such as client files in accordance with established procedures. Serve as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities; orient new staff,students or volunteers; provide related assistance as required. Assist in preparing units for occupancy, which may include moving furniture. What You Bring Excellent understanding of recovery, harm reduction and psychosocial rehabilitation principles acquired through a relevant degree in human services, coupled with at least three years of experience working with this population. Knowledge of issues faced by adults with mental illness, addictions, physical health concerns, and homelessness. Understanding of the therapeutic relationship, with the ability to negotiate goals and priorities with clients. Knowledge of local community agencies and other professional services available for the client group. Experience working as part of a team of professionals. Possess a valid driver’s license and be insurable on agency vehicles. Excellent interpersonal, communication and counseling skills for engaging clients in highly personal areas, such as developing healthy living skills and maintaining interpersonal relationships. Good intake assessment,



crisis intervention, advocacy and problem‑solving skills. Computer literacy sufficient to produce and maintain files, records or statistical information. Good analytical, time‑management and organizational skills to contribute to program planning and to coordinate and follow up on issues or activities with clients. Work Setting Ability to clean and prepare units for occupancy, which may include moving furniture. Work involves residents’ homes, so staff may be subjected to second‑hand smoke while performing duties. What We Offer E‑learning program available all year round.
