Support marginalized populations as an Outreach Worker at Sheldon M Chumir Health Centre. Provide vital case management and promote health while working collaboratively with health professionals and community partners.
This part time position, located in Calgary, requires a dedicated individual to manage outreach assessments and create tailored treatment plans. You will advocate for clients, connecting them with essential services and supporting their daily living skills, ensuring they can stabilize in their environments. Your role includes crisis stabilization, observing client progress, and collaborating closely with multidisciplinary teams to enhance health outcomes.
Key Responsibilities:
• Conduct outreach assessments to establish service goals
• Collaborate on outreach treatment plans with clients and clinicians
• Provide education on daily living skills and activities
• Observe and report on client behavior and progress
• Offer crisis stabilization support as needed
Requirements:
• Diploma in a related field from a post-secondary program
• 1-3 years’ experience with marginalized populations
• Excellent communication skills, both written and verbal
• Capacity to manage complex clients independently
• Knowledge of Truth & Reconciliation Commission principles
