the successful candidate will be responsible for providing clinical care to support both paediatric and adult patients with Sickle Cell Disease. The nurse will work in both the paediatric and adult Sickle Cell Disease clinics with the interprofessional teams to provide seamless care for patients with Sickle Cell Disease. The nurse will engage in roles to enhance care pathways in the ED, inpatient, and clinic areas for this patient population with follow-up with specialists and primary care providers. The ideal candidate will have a commitment to learning about inequities and social issues encountered by patients with Sickle Cell Disease. As a Registered Nurse, you will practice according to the current standards of practice of the College of Nurses of Ontario, the Standards of Nursing Practice, policies and procedures of SHN. You will also provide high quality nursing care to patients and their families, based on the philosophy and conceptual framework of Nursing of SHN.
