Key Responsibilities:
• Provide comprehensive care in an interdisciplinary team
• Support team delivery of client-centered care




• Maintain communication with other care disciplines
• Participate in interdisciplinary team huddles and updates
• Report client condition changes to the primary care team
Requirements:
• OTA/PTA diploma from an accredited program
• Minimum one year of relevant experience preferred
• Experience in home/community or acute care
• Valid driver's license and vehicle for travel
• Robust communication and organizational skills
