Full-time Permanent Position, 35 hours per week Salary Range: $82,308 to $99,164, plus Benefits and HOOPP Pension We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction and homeless programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services. Regent Park CHC is a hub partner for the Downtown East Toronto’s Homelessness and Addiction Recovery Treatment (HART) Hub. The HART Hub is intended to address locally identified gaps in mental health, addictions, primary care, supportive housing and social services by leveraging an integrated approach and co-locating services both in office and proximate to the HART Hub site at Regent Park CHC. We currently have the following opening on our HART Hub team. Position Summary The Project Manager is responsible for supporting the implementation and ongoing coordination of the Downtown East Toronto HART Hub program, working closely with internal teams and external service delivery partners to advance program objectives. In addition, the role supports the Regent Park Community Health Centre leadership team on organization-wide initiatives that span multiple departments and contribute to service improvement, system integration, and strategic priorities. The Project Manager will have access to confidential and sensitive information, including funding proposals, budget development materials, and internal planning documents. The role requires a high degree of discretion, sound judgment, and the ability to work independently when coordinating multi-agency partnerships and supporting initiatives that may influence program delivery, resource allocation, and organizational decision-making.



Responsibilities Include HART HUB Build and maintain strong relationships with HART Hub partners including front-line staff, community agencies, clients, and funders. Facilitate inter-agency governance structures, working groups, and committees in a leadership capacity. Coordinate all program reporting for funders, the Ontario Health Team (OHT), and RPCHC Executive leadership. Draft and revise governance documents, including Terms of Reference and MOUs, ensuring alignment with funder and strategic expectations. Contribute to the development of communication strategies and messaging related to program operations and implementation. Plan and facilitate meetings with working groups, steering committees, and Executive partners. Prepare agendas, meeting materials, and minutes. Identify, elevate, and address partnership or operational issues that may affect program success or strategic direction. Other Lead initiatives to develop and implement annual work and project plans that align with RPCHC’s strategic priorities. Conduct environmental scans and research to inform project scope and deliverables. Coordinate all aspects of assigned projects, including timelines, resources, partner engagement, and deliverables. Create, update, and track implementation of project plans using project management tools. Perform qualitative and quantitative data analysis to advise on program direction and resource use. Access and use confidential data and strategic planning documents to inform decision-making and reporting. Ensure that project objectives and KPIs are aligned with the strategic plan and funding agreements. Prepare reports and presentations for internal and external stakeholders, including funders and executive leadership.



Project Management Professional (PMP) certification, or an equivalent recognized project management certification, is required. Post-secondary degree/diploma in related field e.G., health administration, information technology, business administration; Master of Public Health, Public Policy, Health Administration is an asset. Minimum of 3 years of demonstrated leadership and management of complex multi-stakeholder projects, preferably in a community health or other healthcare setting. Strong knowledge of and experience leveraging design thinking principles and lean methodology is an asset. Certification of Project Management Principles is an asset.
