Job Overview

We are currently seeking Social Care Worker to join Xpress Health. This is a part time role based in westmeath.

Key Responsibilities

- Deliver day-to-day care, supervision, and support to service users in residential or community settings.
- Develop and implement person-centred care plans in line with HIQA and Tusla standards.
- Support service users in education, employment, independent living, and personal development.
- Advocate for service users’ rights, ensuring their voice is respected in decision-making.
- Maintain accurate records, reports, and care documentation.
- Administer medication and follow health, safety, and safeguarding protocols.
- Collaborate with families, external agencies, and multidisciplinary professionals.
- Manage challenging behaviours using approved therapeutic interventions (e.g., MAPA/CPI).
- Participate in supervision, team meetings, and ongoing training.
