As a Nurse Practitioner , you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient‑centred care – and be supported by a collaborative team that includes over 9,100 regulated health care and other professionals.

Responsibilities

- Ensure effective delivery of Nurse Practitioner services for the Palliative Care Consultation Team’s patients and community patients unable to access care through a family physician, clinic, FHT, CHC, etc. upon referral from Ontario Health atHome and in accordance with professional college standards, Ministry Guidelines and Ontario Health atHome policies.
- Diagnose, treat, evaluate, consult and manage complex pain and symptom and associated chronic disease management issues during the patient’s palliative trajectory, in order to delay/prevent institutionalization and facilitate reintegration into the community.




- Provide services across various care sectors including home, hospital, retirement homes and long term care.
- Work collaboratively with Hospice as a member of the Palliative Care Consultation Team (PCCT), reviewing and completing assessments to assist in providing recommendations and consultative guidance to support challenging palliative symptom management and diagnoses.
- Travel for this position between all sites of the Ontario Health atHome in the Erie St. Clair region; potential to work out of Hospice sites.

Required Qualifications

- Registered Nurse, Nurse Practitioner with current registration with the College of Nurses.
- Working knowledge of community resources and health care professionals’ roles.
- Good knowledge of Ontario Health atHome priorities, policies, practices and service standards.
- Positive knowledge and understanding of legislation governing health care activities.
- Knowledge of direct care/care coordination models used in community health care organizations.
- Specialized education in palliative pain and symptom management including a minimum of Fundamentals and CAPCE. LEAP certification will be considered an asset.
- Mandatory COVID‑19 vaccination policy; proof of vaccination required prior to start date.
- Valid Ontario Driver’s License and access to a vehicle.
- Ability to operate a computer.

- Demonstrated proficiency in Canada’s two official languages will be looked upon favourably.

- Anticipated Start Date: September 14, 2026.
- Position Status: Full‑Time (Monday to Friday 8:30 am – 4:30 pm).
- Hours of Work: 37.5 hours per week.
- Location: Primary area will be Chatham – occasional support to Windsor & Sarnia areas.
