Key Responsibilities:
• Provide specialized care in Peritoneal Dialysis
• Engage in patient and family education regularly
• Conduct clinical assessments and chart accurately




• Maintain effective communication with clients and team
• Support management in training and mentoring programs
Requirements:
• Registered with CNO and in good standing
• Experienced in Home and Community Care preferred
• Valid Driver's License and reliable vehicle required
• Current BCLS certification (Level C)
• Clear Vulnerable Sector Check, updated TB test
