Position Details Position #: 9008108 Expected Start Date: June 01, 2026 Union: HSAS Facility: Home Care Office - Melfort City/Town: Melfort Department: Case Management Type: Full-time regular FTE: 1 Shift Information: Days, Standby Number of Hours per Rotation: 28 shifts of 8 hours per 6 weeks Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Assessor Coordinator - Degree Senior $39.658 to $48.494 Travel Required: No Human Resources Exemption: No Experience - Two (2) years clinical experience Education - Diploma/Degree in Nursing or BSW, Physiotherapy, or Occupational Therapy Degree Licenses - Valid Class 5 driver's license Other Education and Training - Bachelor of Occupational Therapy is acceptable - Bachelor of Nursing is acceptable - Bachelor of Physical Therapy is acceptable - Bachelor of Social Work is acceptable - Undergraduate degree in Health or Social Sciences Competencies - Intermediate - Time management skills - Basic - Leadership skills - Basic - Conflict resolution skills - Basic - Interpersonal skills - Basic - Verbal communication skills - Demonstrates leadership skills - Basic - Problem solving skills - Advanced - Computer skills - Basic - Organizational skills - Public relations skills - Data analysis skills - Basic - Writing skills - Basic - Communication skills - Leadership skills such as engagement, communication,

- Knowledge of Occupational Health and Safety Standards and application of such - Articulates theory and knowledge related to patient and family centered care - Ability to be self directed and work relatively independently - Demonstrated ability in program development, implementation and evaluation - Ability to establish and maintain interpersonal/consultative relationships - Ability to work as a member of a multi-disciplinary team - Ability to work under stress - Ability to apply research to clinical practice - Demonstrated commitment to continuing career development - Positive approach to deal with challenging situations with empathy and sincerity - Makes judgements on the basis of critical thinking and problem solving criteria - Ability to work with minimal supervision - Ability to work with people with professionalism and respect - Ability to develop and implement programs / resources from needs assessments - Demonstrates ability to accept and implement change Other Information - Experience using Procura and MDS-RAI assessment tools is considered an asset - Registered/eligible for registration with the appropriate licensing body - Access to a reliable vehicle
