Join St. Leonard’s Community Services as a Community Case Worker for the Reintegration Program on an 18-month contract. Play a key role in supporting reintegration for individuals involved with the criminal justice system.
In this position, you will manage assessments, case planning, and provide essential day-to-day support. As a vital member of the team, you will create individualized support plans while fostering community connections for program participants. You'll also oversee clinical assessments and collaborate with other community partners to ensure effective service delivery.
Key Responsibilities:
• Develop and implement individualized support plans
• Conduct assessments of risk and clinical suitability
• Network and build partnerships for service delivery




• Represent SLCS and liaise with ministry and court officials
• Provide crisis management and advocacy for participants
Requirements:
• Post-secondary education from an accredited institution
• 1-3 years of relevant experience in social services
• Valid G Class Driver’s License and reliable vehicle
• Intermediate First Aid/CPR certification
• Proficiency in both official languages is an asset
