- Maintain an independent panel of members, providing comprehensive care
- Develop individualized care plans and oversee referrals




- Support clinical protocol and governance implementation
- Mentor Associate Physicians and allied health providers
- Assist with regulatory compliance and quality assurance Requirements:
- Medical degree with residency in Family or Internal Medicine
- 3-7 years of primary care experience
- Full licensure with CPSO in Ontario
