Job Summary
Fraser Health is currently hiring Casual Licensed Practical Nurses to join the Access, Care & Transitions (ACT) team at Queen’s Park Care Centre in New Westminster. This is a excellent opportunity for LPNs looking to build experience in a supportive and collaborative healthcare environment while making a meaningful impact on patient care and transitions. You’ll work alongside an experienced interdisciplinary team focused on providing compassionate, patient-centered care.

Detailed Overview
In accordance with the Licensed Practical Nurse (LPN) Standards of Practice Framework, the Licensed Practical Nurse works collaboratively as a member of the ACT team. Utilizing multiple clinical information systems such as Gold Care, Strata Pathways, Excel, and Paris, the ACT LPN reviews and processes referrals and client assessments and assists with the allocation of Fraser Health non-acute bed resources such as long‑term care, assisted living, hospice, and convalescent care; ensures access for eligible clients at highest need and respects client and family needs as well as the characteristics and strengths of the individual homes.

Responsibilities

Receives, reviews and processes referrals and assessments forwarded to Access, Care and Transitions (ACT); utilizes applicable clinical information systems.

Maintains and ensures timely communication of all current information regarding care needs for clients in various assigned areas, based on urgency.

Assists the ACT Coordinators with setting priorities for bed allocation across Fraser Health based on client and system needs. Facilitates admissions by matching eligible clients at highest need with the appropriate care home, considering client/family needs and the characteristics and strengths of the individual homes,



and by acting as a liaison between referring care teams and receiving care homes or hospice.

Supports the management of the Long‑Term Care Access Waitlists in accordance with procedures aligned with the Ministry of Health Long‑Term Care Access and Fraser Health policy by ensuring the prioritization guidelines are followed during the matching process.

Acts as a resource and liaison to internal/external staff, agencies, clients and families regarding information and issues related to long‑term care. Provides input into written information and shares information with clients, families and other healthcare professionals regarding access issues.

Identifies health and safety concerns and escalates to the ACT Coordinator, Team Leader and Manager as required; reviews the care planned or provided and provides input into appropriateness of care; arranges for further care needs assessments as required; completes site visits as necessary.

Facilitates transfers as appropriate and in collaboration with the ACT Coordinator, ensuring changes are reflected in the applicable system; works with appropriate agencies and health authorities.

Completes and maintains complete documentation of client information and records such as transfer lists, forms, records, statistical information and other related information, as required, and in order to support optimum bed utilization.



Works with clerical support staff to maintain databases and transfer lists and participates in the collection of data related to non‑acute bed resource utilization.

Provides input into the development and revision of long‑term care policies for improved care/clinical practice protocols, system responsiveness in long‑term care homes, and improved client access and transition.

Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor as appropriate.

Performs other related duties as assigned.

Qualifications
Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year of recent, related experience in long‑term care, or an equivalent combination of education, training and experience.

Skills and Abilities

Knowledge of gerontology, geriatrics and the needs of adults living with chronic illness.

Ability to complete client assessments.

Knowledge of the broad health care services, community resources and the continuum of care.

Knowledge of relevant acts, legislation, standards and policies governing the operation of long‑term care homes.

Knowledge of and ability to utilize applicable clinical information systems (e.g. Gold Care, Strata Pathways, Excel, Paris).

Ability to type 40 wpm.

Ability to promote client‑focused care including sensitivity to diverse cultures.

Ability to communicate effectively both verbally and in writing.

Ability to work effectively in a dynamic environment with changing priorities.

Ability to work independently and as a member of an interprofessional team.

Physical ability to perform the duties of the position.
