As a Street Outreach Worker, you will engage with community members to create actionable Service Plans while connecting them to culturally appropriate resources. Key Responsibilities:
• Bridge service provision with community First Nations
• Create and implement Service/Action Plans for clients




• Conduct self-assessments for individual pathways
• Facilitate community support and skills training
• Ensure access to holistic support services

Requirements:
• Preference for registered First Nations members
• Career or diploma in Social Services Work
• 2-5 years experience with Indigenous populations
• First Aid and CPR certifications mandatory
• Valid Class 'G' Driver's License and vehicle required
