Step into a rewarding role as a Community Liaison Worker with Vancouver Coastal Health at the Lily Lee Community Health Centre in Vancouver, BC. Engage with clients facing mental health and substance use challenges.

As part of the Groundspring Primary Care Team, you'll contribute to an integrated service delivery model. Your responsibilities will include advocating for clients, coordinating care, and facilitating daily living support. Key Responsibilities:
• Advocate for client needs within the healthcare system
• Document and monitor client interactions and developments
• Facilitate educational groups for clients and families




• Assist clients with daily living activities and appointments
• Liaise with healthcare professionals for coordinated care

Requirements:
• Certificate from a recognized Health Care Worker program
• At least two years of relevant experience
• B.C. Driver's license and personal vehicle access
• Familiarity with mental health issues and cultural competence
• Ability to manage client crises and conflicts effectively
