Act as a Community Liaison Worker with Vancouver Coastal Health at the Inner City Integrated Care Team. This position supports women's health needs through integrated community services in Vancouver, BC.

This role emphasizes interaction with clients facing mental health and substance use issues, advocating for their needs and facilitating access to resources. You will work with a dedicated team, coordinating care and planning group sessions to foster therapeutic relationships and client growth.

Key Responsibilities:
• Provide client support in health and daily living tasks
• Facilitate connections with community services and resources
• Document client interactions and health observations
• Co-lead client groups and case planning activities




• Collaborate with interdisciplinary health teams

Requirements:
• Certificate in Community Health Work and 2 years’ experience
• Valid B.C. Driver's license and vehicle for business
• Understanding of harm reduction and mental health practices
• Ability to communicate effectively with clients and colleagues
• Experience working with culturally diverse populations
