Provide compassionate rehabilitation as an Occupational Therapist with St. Joseph's Home Care. Focus on adult clients in community settings, helping them regain independence after hospital stays.

This role involves assessing client needs and crafting personalized treatment plans to support their journey back to functional independence. You'll be part of an interdisciplinary team dedicated to offering secure, effective, and meaningful therapy. Your contributions are vital in the transition from hospital to home for clients facing complex medical challenges.

Key Responsibilities:
• Complete holistic evaluations of client capabilities
• Create and implement individualized therapy plans
• Teach clients and families about therapeutic objectives




• Work with teams for smooth care transitions
• Mentor and supervise OTA/PTA personnel

Requirements:
• Bachelor’s or Master’s Degree in Occupational Therapy required
• Registration with College of Occupational Therapists of Ontario
• 1-3 years of clinical experience in therapy
• Familiar with electronic medical records systems
• Current BCLS or HCP certification
