These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient.
