As a Client Care Attendant, you will support the Reactivation Care Centre’s mission by delivering fundamental care and assisting patients in achieving their reactivation goals. This role involves close collaboration with Registered Nurses to document patient progress and conditions accurately. Your proactive approach to patient needs will ensure a safe and thriving space for recovery.

Key Responsibilities:
• Adhere to KARDEX for personalized patient assistance
• Follow individualized care plans addressing specific health needs




• Report notable patient condition changes to nursing staff
• Address psychosocial needs and maintain open communication
• Document care actions following established protocols

Requirements:
• Valid registration with the College of Nurses of Ontario
• High school diploma or equivalent education required
• Current registration with HSCPOA preferred
• Certification in Personal Support Worker from an accredited program
• At least one year of full time experience in care
