Become an essential part of the La Ronge Health Centre as a Nurse B Clinician focused on patient case management. This full time temporary role is pivotal for care coordination in healthcare.

Reporting directly to the Primary Health Nursing Coordinator, you will provide holistic healthcare solutions for patients at Pinehouse Health Centre. The RN Case Manager will engage in outreach and programming while emphasizing the management of communicable diseases. Effective communication and relationship building with patients, their families, and the healthcare team will be central to your role.

Key Responsibilities:
• Oversee patient case management with a healthcare team
• Maintain therapeutic relationships with patients and families
• Conduct thorough health assessments and referrals




• Educate and advocate for patient health needs
• Support public health initiatives in community settings

Requirements:
• Minimum of two years’ experience in health care
• Bachelor’s degree or nursing diploma required
• Registered with the College of Registered Nurses of Saskatchewan
• PALS, BCLS, ACLS certifications are mandatory
• Capability for transferring medical functions as per CRNS
