As the Care Partnerships Manager, you will lead initiatives to formalize and manage partnerships across various health institutions, ensuring the integration of culturally safe practices. Key Responsibilities:
• Establish referral networks with hospitals and community clinics
• Negotiate partnership agreements and manage workflows

• Facilitate onboarding for Mino Care's teams at partner sites
• Monitor operational metrics and quality improvements

Requirements:
• 3-5+ years in health system coordination or partnership management
• Strong knowledge of provincial and federal health systems
• Experience with hospitals, midwifery, or social services
• Commitment to equity-centered, culturally safe health practices
• Excellent team-oriented and negotiation capabilities
