Department: Transitional Care Program

Reports to: Manager, Program Services

Hours: 22.5 hours per week

Wage Range: $38.42 - $39.02 per hour (Start rate non-negotiable)

Shifts: 0715-1915, 1915-0715 – Rotating Days/Nights/Weekends/Holidays (subject to change)

Status: Permanent Part-Time

Closing Date: July 6, 2026

Position Summary
As a member of a multi‑disciplinary team, the Registered Practical Nurse practices in accordance with relevant legislation and standards, the UHN Professional Practice Model and the mission and vision of nursing at UHN to provide compassionate, collaborative and competent care while meeting the legal and ethical requirements of the profession.

Responsibilities

Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of an interdisciplinary practice.

Use current theoretical and research knowledge and input of team members to plan and implement care for patients and families.

Participate in inter-professional team care planning forums, inter-professional rounds and family conferences.

Provide mentorship to RPN students and new staff on the unit.

Participate in professional, program and hospital-wide initiatives, including professional development of self.

Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures to ensure a safe environment for patients.





Qualifications

Valid registration with the College of Nurses of Ontario.

Diploma in Practical Nursing.

Recent related clinical experience preferred.

Effective problem recognition, problem solving and decision‑making skills.

Demonstrated commitment to continuing professional development.

Demonstrated commitment to team success.

Resilience and effective coping strategies to manage change.

Demonstrated computer literacy in word processing and electronic patient record.

Working knowledge of the College of Nurses of Ontario standards of practice.

Evidence of satisfactory attendance and punctuality.

Current UHN employees must have successfully completed their probationary period, have a good employee record and satisfactory attendance in accordance with UHN's attendance management program.
