You will operate as part of the SAFER Program, delivering impactful healthcare services in a community setting. With a commitment to trauma-informed care, you will assess, screen, and educate participants while collaborating effectively with an interdisciplinary team. This role emphasizes a holistic yet individualized approach to healthcare, fostering improved outcomes for those affected by stigma, criminalization, and poverty.
Key Responsibilities:
• Address complex healthcare needs through individualized support
• Conduct necessary screenings and assessments for participants
• Collaborate with partners to provide seamless care
• Share knowledge and resources that empower participants in their health
• Involve in team meetings and community-building activities
Requirements:
• Valid Licensed Practical Nurse designation registered with BCCNM
• 1-5 years' full-scope nursing experience
• Familiarity with harm reduction, mental health, and substance use services
• Commitment to diversity and equity-focused initiatives
• Solid communication and advocacy skills in patient interactions
