As a Full Scope Licensed Practical Nurse, you will play a vital role in improving patient outcomes. This position involves working independently to assess client needs and designing care plans tailored to fostering independence and confidence in their health journeys. Key Responsibilities:
• Establish rapport with clients through effective communication
• Conduct health assessments and develop care strategies
• Provide direct nursing interventions and follow-ups




• Coordinate care transitions with community resources
• Participate in quality improvement initiatives

Requirements:
• Registered as a Licensed Practical Nurse with BCCNM
• One year of relevant clinical experience required
• Valid BC driver’s license for work-related travel
• Proficiency in assessing and managing elderly patients
• Solid skills in educating clients about health management
