Responsibilities

Assess patient/resident/family strengths and needs across physiological, psychological, socio-cultural, and spiritual domains.

Provide a supportive environment, promote involvement in care decisions, share information, discuss concerns, and respect personal choices.

Contribute to developing and updating individualized plans of care; participate in multidisciplinary care planning meetings and family conferences.

Deliver nursing care according to policies, procedures, technology, and FHA standards; document observations and communicate changes to designated staff.

Administer admissions, discharges, and transfers following agency policies and procedures.





Document and update care information to ensure patient safety and comfort.

Operate and maintain equipment related to patient care (e.g., bathtubs, scales, mechanical lifts).

Maintain a safe and clean environment for patients/residents and staff.

Assist in orienting recent staff, students, and other Health Care Team members.

Participate in quality improvement activities, education sessions, team conferences, meetings, and committees.

Perform other related duties as assigned.

Qualifications
Education and Experience
Graduate of a recognized practical nursing program or equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Skills and Abilities

Ability to communicate effectively, both verbally and in writing.

Ability to work with others effectively and to organize work and implement a plan of care.

Knowledge and competencies to work at full scope of practice as required by BCCNM, including administering medications.

Ability to operate related equipment and physically carry out the duties of the position.
