Role Overview
We're seeking Primary Care NPs to provide on‑site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.

What You'll Do

Routine & urgent assessments; chronic disease management

Post‑discharge follow‑ups, medication reconciliation, labs/diagnostics

Coordinate with facility staff, specialists, families/POAs

Same‑day documentation; CMS/regulatory compliance

Schedule & Setting
Full‑time position with a structured weekday schedule. Providers deliver on‑site care across Stockton, Modesto, and Amador with strategically clustered routes, so you spend more time with patients and less time on the road.

Requirements

Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS

Geriatrics or facility‑based experience preferred; EMR proficiency

Perks

Annual compensation range: $140,000-$175,000 (based on experience, schedule, and patient volume)

Additional earning opportunities available based on coverage and panel needs

W-2 with full benefits available; mileage reimbursed per policy
