Description
We are seeking a compassionate and reliable Registered Practical Nurse (RPN) to join our community visiting team. In this role you will provide high-quality, patient-centered care in clients’ homes, including medication administration, wound care, health assessments, palliative care, and chronic disease management. You will work collaboratively with an interdisciplinary team to support clients’ independence, safety and overall well-being.

Shift Details
Evening shifts starting at 4:00 pm: minimum of two (2) shifts per week plus a 1:3 weekend rotation, and statutory holiday rotation. Pay: $39.77 – $43.54 per visit (evenings).

Primary Functions

Provide nursing care within the Community and Clinics.

Organize the day, setting priorities based on continuity of care, the type of care to be given, and the client’s/family’s health needs.

Develop, implement, evaluate, and modify the plan of care outlining the strategies, orders, and actions to achieve expected/desired outcomes.

Assess the health status of the client.

Administer IV therapy, including central line and peripheral line maintenance (IV, pump skill, CVAD skill).

Administer medications topically through natural body orifices, through tubes, and by injection.

Handle wound care, acute and chronic, for certain types and stages of wounds.

Perform catheter care and maintenance.





Ensure that the laptop is synchronized with the network two times a day (at the beginning and end of your shift) to receive information on and update client records.

Check voicemails and fax mail on days scheduled to work.

Attend team/POD meetings as required.

Position Requirements

Minimum 1 to 2 years of clinical practice in a community environment preferred.

Current registration with the CNO.

Current CPR certificate.

Excellent interpersonal, communication, and teaching skills.

Palliative experience preferred.

Ability to administer medications.

Demonstrated initiative and commitment to professional development and skill enhancement.

Valid driver’s license and access to a vehicle.

Must be legally eligible to work in Canada.
