Family Physician – 360° Care+ Program
Location: Vancouver, BC
Type: Full Time or Part Time (minimum 3 days per week)

Position Summary
We invite you to join our dynamic and continuously evolving team at WELL Health as a Family Physician within our 360° Care+ Program. This in‑clinic position provides primary care services and requires on‑site work at our clinic in Vancouver, BC.

Responsibilities

Provide full‑scope family medicine care within the 360° Care+ Program

Perform all required duties as a primary care physician

Provide ongoing coverage when needed

Diagnose, treat, and manage urgent, acute, and chronic health concerns

Conduct preventive health assessments and ongoing wellness management

Order, interpret, and act on diagnostic and screening tests

Prescribe medications and therapeutic interventions

Perform minor procedures within scope

Collaborate with a multidisciplinary health care team to ensure cohesive patient management

Participate in team building activities when schedule allows

Uphold all WELL Health core values (Team, Care, Growth & Integrity)

Maintain accurate, timely patient documentation in the EMR

Participate in patient follow‑up, result communication, and care planning





Support program development and continuous improvement within 360° Care

Qualifications

Licensed and in good standing with the College of Physicians and Surgeons of BC (CPSBC); member of CMPA required

CCFP designation preferred

Strong clinical experience in primary care, including acute, chronic, and preventive health management

Experience or interest in lifestyle medicine and longevity medicine

Comfortable working in a cooperative, multidisciplinary environment

Excellent communication skills with patients and team members

Computer literacy and comfort with EMR systems

Confident, approachable, and committed to delivering an exceptional patient experience

Salary falls within a defined range and will be determined based on the candidate’s experience, qualifications, skills, and the organisation’s needs.
