Job Summary This is a casual position with the possibility of part-time. Registered Practical Nurse is a member of the home care team who works under the supervision of the Registered Nurse and performs various tasks based on the client's plan of care.
Client Care Management Performs assessments in the community ( including nursing)
Ensures standards of care, compliance with College of Nurses standards, state/provincial regulations and standards of care
Develops standards that ensure safe and effective services to clients and families;
Establishes care plan, completes and maintains related records and documentation
Ensures a high standard of customer service is delivered to clients from intake to client conversion,



case setup- up and delivery of service;
Ensures that the level of care established for clients is safe and appropriate based on client and home assessment
Directs the implementation of service goals and objectives
Ensures the evaluation of client care services
Achieves/exceeds client satisfaction, investigates issues, and follows up promptly;
Qualifications Registered with the College of Nurses of Ontario (CNO) in good standing
Experience in doing assessments in Home and Community Care ( 1-2) years
Current BCLS certification (Level C)
Demonstrated clinical assessment and patient/family teaching skills
Proven excellent interpersonal skills and effective oral/written communication skills
Demonstrated excellent organizational, time management, prioritization problem-solving, and conflict-resolution skills
Demonstrated planning and coordination abilities
Proven ability to work effectively as part of a team
Demonstrated ability to work with minimal supervision
Requirements Explicit & updated Vulnerable Sector Check (within the last six months)
Valid Driver’s License & reliable car
Up-to-date TB test within the last four years
High level of customer service experience
Proven interpersonal, organizational, written, and verbal communication skills.
Ability to work independently in a fast-paced, diverse, and multicultural environment.
Demonstrated commitment to providing the highest level of satisfaction in client and staff service care
Schedule Ability to see clients when needed. The ideal candidate is someone who has a part-time job and is able or willing to work casually
