Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health,



and working collaboratively and in partnership with the interprofessional care team in the provision of person‑centered care, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in an acute rehabilitation unit.

Responsibilities

Assess patients within an acute rehabilitation setting, collect information from a variety of sources, make a nursing diagnosis, and determine whether the condition can be improved by appropriate nursing intervention. Plan, perform, and evaluate nursing interventions and care.

Collaborate with patients and their families/caregivers in developing the care plan, encouraging family participation, and ensuring patients’ rights to privacy, dignity, and access to information.

Participate in the care and discharge planning process, ensuring the care plan is established and implemented effectively, updating the plan and intervening as appropriate.

Coordinate nursing care, set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols.

Collaborate with members of the interprofessional care team—including Physicians, Licensed Practical Nurses, Patient Care Coordinators, Clinical Nurse Educators, and other health care professionals—to identify and resolve issues, adjust care plans, and respond to changes in patient condition.

Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance, assessing learning needs and providing information and demonstrations of care activities.

Plan and discuss referral information with patient and family, and initiate referrals to appropriate health care/community agencies.

Document assessments, nursing diagnoses, observations, interventions, outcomes,



and referrals according to practice standards and Fraser Health procedures.

Participate on nursing committees, care conferences, and councils as assigned.

Participate in research and quality improvement activities, identifying patient care issues, collecting data, and recommending changes to practices and protocols.

Participate in and attend in‑services and other educational programs, maintaining current clinical competence and knowledge within the area of practice.

Act as a preceptor or mentor to assigned new nursing staff and students, sharing information and participating in orientation.

Provide feedback to the Manager/delegate regarding the effectiveness of patient care programs, policies, and equipment.

Perform other related duties as required.

Qualifications
Education and Experience
Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year recent related acute care experience, or an equivalent combination of education, training and experience.

Skills and Abilities

Complete initial and ongoing patient assessment and provide nursing care through appropriate interventions.

Knowledge of rehabilitation nursing in an acute care setting.

Communicate effectively with patients, families, the public, medical staff, and interprofessional team members using verbal, written and electronic means.

Promote person‑centered care that demonstrates empathy, sensitivity to diverse cultures, and patient advocacy.

Integrate and evaluate data from multiple sources to problem‑solve effectively.

Teach and instruct patients and families about essential health topics.

Organize and prioritize work.

Collaborate and promote cooperation among interprofessional team members.

Use factual information, prior learning and basic principles and procedures to support nursing decisions with research‑based evidence.

Operate related equipment, including relevant computer applications.

Physical ability to perform the duties of the position.
