Conduct intake assessments and determine service needs
Coordinate and implement personalized support plans
Monitor ongoing case management and discharge planning
Collaborate with service providers for effective program delivery
Travel to various locations for client assessments Requirements:
Licensed Skilled in Nursing, Social Work, or related field
Minimum five years of relevant experience required
Excellent assessment and negotiation skills essential
Valid driver’s license for site visits
