Key Responsibilities:
• Facilitate client check-ins and monitor wellness
• Identify and respond to crisis situations effectively
• Collaborate with team members and external service providers
• Support daily living skills such as budgeting and meal prep
• Document client interactions promptly and confidentially
Requirements:
• Relevant Diploma or Degree in Social Services
• 3 years experience in housing instability and support
• Expertise in crisis intervention and problem-solving
• Familiarity with community health resources
• Valid First Aid/CPR certification and Police check
