Job Description
The Social Worker BSW will work with the Continuing Care team to create an workplace that protects the financial security and quality of life of residents. The Social Worker BSW will collaborate with Continuing Care and Home Care teams and families, clients and residents to establish a plan to meet unmet needs in areas of financial security and advanced care planning. The role includes ethical decision making to identify, assess and resolve concerns or problems expressed by the referral source, collaborating with family, client and/or health care team.
Focus will be on financial needs and addressing social determinants of health. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.
Education
Bachelor of Social Work (BSW)
Licenses
Valid Class 5 driver's license
