Make an impact on patient health in your role as a Registered Dietitian at SE Health. This position offers flexible, hybrid work providing critical support to clients transitioning from hospital care.

SE Health is looking for motivated Registered Dietitians to be integral to their Acute Care Transitions Program. You will engage in collaborative care planning alongside a diverse care team, focusing on the comprehensive needs of each client. The role encompasses conducting assessments and implementing client-centered treatment interventions to optimize recovery and enhance quality of life.

Key Responsibilities:
• Conduct comprehensive assessments of nutritional needs
• Establish collaborative care goals and implement treatment
• Engage effectively with interdisciplinary rehabilitation professionals




• Work flexibly in hybrid settings, including home visits
• Stay organized and maintain effective communication with clients

Requirements:
• Experience in acute care or community rehabilitation preferred
• Robust interpersonal and organizational communication skills
• Must have a positive attendance and reliability record
• Must possess a valid driver’s license and access to a vehicle
• Ability to adapt to new software and applications
