Our client is seeking a Nurse Practitioner (NP) to join their team providing primary care and addiction services to individuals impacted by homelessness and substance use.
Key Responsibilities
Provide primary care and addiction services using a client-centered approach
Conduct assessments, diagnosis, and treatment for clients with complex physical and mental health needs
Collaborate within an interprofessional team and make community referrals as needed
Maintain accurate EMR documentation and support quality improvement initiatives
Promote evidence-based practices and contribute to clinical protocols and policies
Qualifications
Master’s Degree in Nursing (MSN) and Primary Health Care NP (NP-PHC) certification
Current CNO registration (Extended Class)
1+ years’ NP experience in primary care preferred
Experience supporting individuals facing homelessness or substance use
Valid Ontario driver’s license and access to a vehicle
CPR certification and vulnerable sector police check required
Flexibility to work across multiple sites, including one evening per week

Pay: $130,000.00-$135,000.00 per year
