Alberta Kidney Care North – Renal Social Worker II in Grande Prairie is part of an interdisciplinary team delivering psychosocial services to patients and families dealing with kidney disease. The role includes assessment, counselling, resource navigation, case management and discharge planning in a busy clinical setting.
We seek a Bachelor of Social Work with ACSW registration eligibility, 1–2 years’ renal care experience, solid communication and teamwork, and BLS certification.
