Description

Permanent part‑time Long Term Care (LTC) Practitioner position with St. Vincent’s Langara, a 196‑bed long term care home in Vancouver. The role expands a family practice by providing challenging, diverse, and impactful care to frail elderly residents.

Responsibilities

- Assist with resident caseload for the LTC home, providing comprehensive, resident‑centered care.
- Participate in interdisciplinary team meetings with physicians, nurse practitioners, nurses, allied health and leadership.
- Meet the LTCI best‑practice expectations: regular proactive visits, on‑site medication reviews, attendance at care conferences, involvement in 24/7 on‑call, and completion of chart documentation.
- Support mentorship and onboarding of current practitioners within the LTCI program.




- Engage in LTC‑specific events, projects, and after‑hours care program as part of the incentive structure.

Qualifications

- Licensed medical practitioner with or willing to obtain Providence Health Care (PHC) privileges and credentials for the care home.
- Experience or willingness to work within long‑term care settings.
- Commitment to the formal LTCI program and its expectations.
