As a Supportive Housing Intensive Case Manager, you will provide vital assistance to families and individuals facing complex challenges related to housing stability. With a robust emphasis on an Indigenous trauma-informed approach, you will develop tailored case plans and coordinate resources for clients experiencing homelessness. You'll be part of a dedicated housing team, committed to delivering comprehensive support within a client-centered framework.
Key Responsibilities:
• Manage a caseload of 6-10 families in Supportive Housing
• Develop individualized case plans based on Housing First principles
• Provide daily support for housing stability to clients
• Assist clients in navigating resource systems and housing applications
• Maintain accurate case files and document client progress
Requirements:
• Diploma in social services, mental health, or related field
• Two years of experience in housing and social services
• Understanding of Indigenous issues related to homelessness
• Excellent communication skills and client advocacy ability
• Current driver abstract and valid driver's license required
