In this role, you will report to the Health Promotion & Community Support Manager. Your focus will include conducting client-centered assessments, facilitating group programs, and managing data for effective service delivery. This position emphasizes an interdisciplinary approach to health equity through community engagement and holistic care.

Key Responsibilities:
• Conduct client-centered intakes and assessments
• Facilitate community health promotion and educational activities
• Build partnerships with local organizations
• Support outreach and communication efforts
• Manage data collection and program coordination

Requirements:
• Post-secondary education in a related field
• Minimum 3 years in community health or outreach
• Robust communication and facilitation skills
• Knowledge of health equity and anti-oppressive practices
• Familiarity with Etobicoke communities is an asset
