Key Responsibilities:
• Conduct accurate resident assessments and maintain care plans
• Provide direct nursing care, administer medications safely




• Participate in care conferences and support transitions
• Coordinate care with physicians and team members
• Educate families about health conditions and care plans

Requirements:
• Current registration as a Licensed Practical Nurse
• Experience in senior or long-term care preferred
• Strong clinical knowledge and communication skills
• Current CPR and First Aid certification required
• Proficiency with electronic documentation
