Transform lives as a Case Manager with CMHA-YRSS in South Simcoe, focusing on mental health recovery. This role supports clients through personalized care and community integration. As part of the Assertive Community Treatment Team, you'll work with clients aged 16 and older dealing with reliable mental illness or addiction.
Your responsibilities will include assessing needs, developing goals, and providing resources that enhance clients' quality of life and support their recovery path. Key Responsibilities:
Manage a client caseload adhering to Ontario’s standards
Employ recovery-focused principles in client engagement
Utilize effective communication to meet client needs
Facilitate community resource access and connections
Participate in advocacy for improved client care Requirements:
Degree or diploma in health/social services
Registration with OCSWSSW or a related body
Minimum one year experience in community settings
Knowledge of mental health legislation
