Shannex is seeking a Licensed Practical Nurse to join its creative Transitional Health team. In this role, you will engage in interprofessional collaboration, applying evidence-based practices to create effective care plans. Your contributions will help patients transition smoothly from hospital care to their communities through focused assessments and personalized support.
Key Responsibilities:
• Perform initial and ongoing health assessments for patients
• Administer and record medications per facility policy
• Assist in developing and evaluating individual care plans
• Ensure timely task completion by Continuing Care Assistants/PSWs
• Provide leadership and training for new team members
Requirements:
• Graduate from an approved Practical Nursing Program
• Current registration with provincial Nursing regulatory body
• Minimum two years of nursing experience required
• Valid CPR & First Aid Certification necessary
• Strong supervisory skills and evaluation abilities
