SE Health is seeking a dedicated Registered Practitioner Nurse in Hensall, Ontario. In this part-time role, you will conduct client assessments, develop individualized care plans, and support transitions in care. You are expected to work from Monday to Friday between 2 PM and 6 PM, providing essential support and engaging with community health services.
Key Responsibilities:
• Conduct detailed client assessments and create care plans
• Act as a primary provider for assigned client caseloads
• Collaborate with community support services
Requirements:
• Current Certificate of Competence with the College of Nurses of Ontario
• Availability for part-time hours, 2-6 PM
• Clear criminal background and updated immunization records
• Willingness to pursue SE Health certifications
• Current CPR certificate required
