Health is looking for a dedicated Registered Nurse to join the Integrated Care Team at our Downtown Community Health Centre. This role centers on providing compassionate care to clients with chronic illnesses and complex health needs, leveraging a blend of direct care and outreach services to optimize health outcomes. Key Responsibilities:
Work collaboratively with an interdisciplinary care model
Address the healthcare needs of clients facing social determinants
Utilize assessment skills to evaluate mental and physical health
Implement care plans and documentation for client progress
Build and maintain respectful therapeutic relationships Requirements:
RN or RPN licensure with BCCNM
Minimum one year of relevant nursing experience
Possess a BC Driver’s License and vehicle access
Solid foundation in community nursing theory
Understanding of mental health policies and practices Impact lives positively through your role in integrated health services at Vancouver Coastal Health.
