In this role, you will be instrumental in providing care to individuals in staffed homes, helping them thrive in their communities. Collaborate with a vibrant team to develop Personal Service Plans and facilitate daily living activities. Your expertise in behavior management and clear communication will be key in advocating for the rights and choices of supported individuals, creating a supportive and inclusive environment.
Key Responsibilities:
• Advocate for the rights and choices of individuals
• Engage with individuals to empower community participation
• Develop and document Personalized Service Plans
• Assist with daily living tasks and medication needs
• Maintain accurate health information records
Requirements:
• Grade 12 education and Community Support Worker Diploma
• 2+ years experience in social services/healthcare
• Current First Aid/CPR and Food Safe certification
• Class 5 driver’s licence and explicit Driver’s Abstract
• Criminal record clearance for working with vulnerable adults
