In this full time permanent position, you will coordinate supports for individuals dealing with vision loss. You're integral in providing assessments, enhancing client experiences, and ensuring they receive timely access to necessary services. Your expertise will create vital connections between clients, VLR staff, and healthcare providers to foster independence.

Key Responsibilities:
• Schedule therapy appointments with primary service providers
• Gather client feedback and implement action plans
• Monitor and follow up for client support




• Identify emerging care options to fill service gaps
• Maintain accurate documentation in client health records

Requirements:
• Client service experience in health or social sectors
• Diploma in Social Services or related field
• Strong communication and interpersonal skills
• Understanding of healthcare navigation and care planning
• Experience with individuals with vision loss
