Vision Loss Rehabilitation Canada (VLRC) is seeking a Client Care Navigator in Victoria, Kelowna, or Abbotsford, BC. This full time role coordinates supports for people living with vision loss, acting as the primary contact after diagnosis and ensuring timely access to VLRC services.
You will manage care plans, liaise with healthcare providers, and document care activities in EMR systems. The successful candidate will have health/service experience and a patient-centered approach.
