Under the direction of the Community Team Manager, the Registered Practical Nurse (RPN) is responsible for the provision of nursing services, including medication management and administration in conjunction with the team, assessing the needs of clients, making appropriate referrals to community physicians and consulting with interprofessional team members. The Registered Practical Nurse also contributes to the development of comprehensive client-centered assessments and treatment plans. In addition, the RPN will assist in the service coordination for an assigned group of clients that includes providing a range of treatment, rehabilitation and support services.
The Community Adult Mental Health Programs aim to prioritize dignity, autonomy, and safe transitions for individuals living with dementia, mental health conditions, acquired brain injuries, substance use health challenges and cognitive impairment or decline. The stepped-care approach matches client needs to community-based support, transitional support, acute inpatient and complex continuing care. This is a comprehensive, wrap-around service aimed at improving person-centered care, reducing caregiver distress, and enhancing continuity of care.
Required Qualifications and Experience Registered Practical Nursing Diploma
Active, unrestricted registration with the College of Nurses of Ontario
Minimum one (1) year of work experience providing services to adults with a serious mental illness and/or neurocognitive changes in an outpatient psychiatric setting
Minimum three (3) years of work experience in an inpatient psychiatric setting, preferred
Valid Ontario G Class driver’s license, satisfactory driver’s abstract and access to reliable means of transportation on a regular basis
Position Duties & Responsibilities Provides individualized service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assumes primary responsibility for overall treatment goals and plans in collaboration with the client and treatment team, providing individual supportive therapy and symptom management,



ensuring immediate changes are made in the treatment plans as clients’ needs change, educating and supporting clients’ families, and advocating for clients’ rights and preferences
Participates in and conducts parts of the client-centered comprehensive assessment of psychiatric history, mental status and diagnosis; physical health and dental health (e.g. health status and any medical conditions present); use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships
Acts as liaison and consult with community agencies and families to maintain coordination in the treatment process
Participates in daily organization of client related activities in collaboration with other team members
Provides on-call crisis intervention covering nighttime hours, serves as backup to evening and weekend staff, and provides night and weekend staff support as required
Takes a lead role or participates in providing medication administration and medical services
Under the direction of the MRP and in collaboration with other nurses on the team, develops, revises, maintains, and leads team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the team psychiatrist; evaluates and charts psychotropic medication effectiveness, complications, and side effects; and arranges for required lab work according to protocol
Assists with ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment. Makes appropriate changes in treatment plan to ensure immediate and appropriate interventions are provided in response to changes in mental status or behaviour, which put clients at risk (e.g. suicidality)




Provides education to clients related to medications, their potential side effects and interactions, and strategies for managing effects
Provides ongoing assessment, problem-solving, side-by-side services, skill training, supervision (e.g. prompts, assignments, monitoring, encouragement), and environmental adaptations to assist clients with activities of daily living
Assists clients to find and maintain a safe and affordable place to live – apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities (e.g. telephone, furnishings, linens, etc.)
Provides individual supportive therapy (e.g. problem-solving, role-playing, modeling and support), social-skill development, and assertiveness training to increase client social and interpersonal interactions in community settings
Assists clients to plan and structure social and leisure-time activities on evenings, weekends and holidays, consistent with the individual placement support model
Contributes to the provision of individual vocational-supportive counseling to enable clients to identify vocational strengths and problems, establish vocational or career goals and plans to reach them, recognizing and targeting symptoms of mental illness that interfere with work
